Submission of this form indicates you have read and agree to the terms of our waiver.

Waiver/Indemnification: As the parent or legal guardian of the participant named herein, I represent that my child is physically fit and able to participate in the activities offered by Rodriguez Development Academy (RDA).

I understand that participation in soccer training and athletic activities involves inherent risks, including but not limited to falls, collisions, physical exertion, and other risks that may result in injury. I voluntarily assume all risks associated with my child's participation in RDA programs, training sessions, and related activities.

I accept full responsibility for and agree to pay any and all medical expenses resulting from injuries sustained by my child while participating in RDA activities. I further agree to release, indemnify, and hold harmless Rodriguez Development Academy, its owners, coaches, instructors, employees, contractors, volunteers, agents, and representatives from any and all claims, liabilities, damages, costs, or expenses arising out of or related to my child's participation in any RDA program or activity.

In the event of an emergency and I cannot be reached, I authorize Rodriguez Development Academy staff, emergency medical personnel, physicians, hospital staff, or other qualified medical professionals to provide medical treatment deemed necessary for my child. I understand that every reasonable effort will be made to contact me prior to treatment whenever possible.

I grant permission for Rodriguez Development Academy to photograph and/or record video of my child during participation in academy activities. I understand that these images and recordings may be used for promotional, educational, marketing, social media, website, and other lawful business purposes without compensation.

By signing below, I acknowledge that I have read, understand, and voluntarily agree to the terms of this Waiver and Release of Liability.
